4 Rs of Fracture Management - An Expert Guide

The 4Rs of Fracture Management

Resuscitate

Reduce

If the fracture is displaced, it will need to be reduced. This means realigning the bone ends so that they are in their normal position. The reduction can be done closed (without surgery) or open (with surgery).

Closed reduction is the most common method of reducing fractures. It is done by applying manual traction and manipulation to the bone ends. In some cases, closed reduction may be assisted by using a local anesthetic or a nerve block.

Open reduction is a surgical procedure that is used to reduce fractures that cannot be reduced closed. During an open reduction, the surgeon makes an incision over the fracture site and then uses surgical instruments to realign the bone ends.

Retain

Once the fracture has been reduced, it needs to be retained in place so that it can heal properly. This can be done with a cast, splint, or external fixator.

A cast is a rigid, molded device that is used to immobilize a fracture. Casts are made of plaster or fiberglass and are held in place with bandages.

A splint is a more flexible device than a cast. It is used to stabilize a fracture and prevent further displacement. Splints can be made of a variety of materials, including wood, metal, or plastic.

An external fixator is a device that is used to hold a fracture in place without the need for a cast or splint. External fixators consist of a series of pins that are inserted into the bone near the fracture site. The pins are then connected to a frame that is used to stabilize the fracture.

Rehabilitate

Once the fracture has healed, the patient will need to undergo rehabilitation to regain the full function of the injured area. This may involve exercises, physical therapy, or occupational therapy.

Exercises are used to strengthen the muscles and improve the range of motion. Physical therapy is a type of exercise therapy that is supervised by a physical therapist. Occupational therapy is a type of therapy that focuses on helping the patient regain the skills needed for daily activities.
